Continue LogoutThe growth in digital health technologies (e.g., mobile apps, wearables, virtual care) is rapidly increasing the volume and variety of real-time patient-generated health data (PGHD). As providers consider strategies to accommodate PGHD into care delivery models, they must make key decisions about how to operationalize the capture, management, and use of PGHD, while overcoming a number of challenges (e.g., mobile data security, patient engagement). Incorporating PGHD data into care management and delivery will determine providers’ effectiveness at managing the health of populations beyond the traditional care encounter.
In this report, we highlight some trends related to PGHD, outline a vision for connected care, cover different use cases, and give some initial considerations for how to approach PGHD initiatives in your organization.
We are now entering an era in which patients can give clinicians data on their daily lives to provide a more complete, holistic picture of health. PGHD allows patients to actively manage their own health and empowers them to participate more in their own health care. In addition, clinicians can use PGHD to get a more comprehensive view of patients’ health beyond face-to-face visits.

Current use cases with PGHD generally address disease management, patient engagement, andprevention and wellness in support of population health management.
