Members across the globe continue to struggle with patient flow: beds are full, patients wait too long in the emergency department, and demand continues to grow. These problems are not new, and most hospitals have spent years working to improve efficiency. Unfortunately, we hear all too often that leaders struggle to sustain improvements, and as a result performance slips.
Effective discharge practices are the bedrock of efficient patient flow, and we have designed a new toolkit to help leaders pinpoint how they can improve discharge processes to embed meaningful change.
This toolkit helps indicate how to sequence activities so that they build on each other. The tools and resources help address common discharge delays in the acute setting, from setting a predicted date of discharge to assembling a care team to proactively plan for patient discharge need.
