The health care market is much more complex—and crowded—today than it was five years ago.
Millions of Americans are shopping through public and private exchanges, Medicare Advantage, and exchange-based Medicaid expansion. They have more control over their health insurance plans than they've had in the past, and the decisions they make during open enrollment—such as accepting higher deductibles and narrower networks in exchange for lower premiums—determine where and how they'll receive care.
