Methodology

For the list, Leapfrog sorted hospitals into four categories and compared them to their peers. The four categories were:

• Children's hospitals
• General hospitals
• Rural hospitals
• Teaching hospitals

To be eligible for recognition, all hospitals were required to:

• Meet Leapfrog's standard for computerized physician order entry
• Meet Leapfrog's standard for ICU physician staffing
• Meet Leapfrog's standard for "never events"
• Meet or make progress toward meeting Leapfrog's standard for billing ethics
• Meet or make progress toward meeting Leapfrog's standard for informed consent
• Report on all applicable measures and meet Leapfrog's standard for 40% to 60% of all applicable measures depending on the type of hospital
