The CMS Interoperability and Patient Access final rule established a new Condition of Participation (CoP) that requires hospitals to make a reasonable effort to send an electronic event notification upon a patient’s admission, discharge, or transfer (ADT). Hospitals must send the notifications to those that need to know of the patient’s status for treatment, care coordination, or quality improvement purposes. This requirement supports broader care coordination initiatives, and improving care transitions is a high priority for CMS because it can help reduce readmissions and overall costs to Medicare.
CMS gives hospitals a lot of flexibility, and many hospitals already send ADT notifications in some way. Each hospital should compare its current capabilities and processes against the requirement to identify any gaps.
