Institutional Special Needs Plans (I-SNPs) are an optional Medicare Advantage (MA) program designed for beneficiaries who have been institutionalized or eligible for nursing home care for 90 days or longer. Post-acute care providers can participate by establishing their own plan, or as a contracted provider for a plan, which can lead to improved financial and clinical outcomes for providers.
