Reducing length of stay with home-based care

    Hospitals often struggle to address complex, geriatric patients’ clinical and non-clinical needs during a single hospital stay, which can increase length of stay and lead to higher rates of readmission.

    To address this, some organizations are creating programs specifically designed to improve clinical outcomes and efficiency for select complex patient populations. While these programs vary, they share a commonality: patients receive some or all of their care at home.
