The Era of the Connected Patient: Integrating Patient-Generated Health Data and Social Determinants of Health

The growth in digital health technologies (e.g., mobile apps, wearables, virtual care) is rapidly increasing the volume and variety of real-time patient-generated health data (PGHD). Simultaneously, the shift toward value-based care is forcing provider organizations to consider the impact of social determinants of health (SDH) on outcomes. In response to these forces, provider organizations must learn to develop mechanisms to capture, analyze, and integrate these new data sources into clinical workflow, while overcoming a number of challenges (e.g., mobile data security, patient engagement).
