    In the 1930s, 40% of patient encounters took place at home. By 1980, it was only one percent. However, recent years have seen a trend towards the resurgence of home-based care—and CMS is currently exploring the care delivery model through the Independence at Home (IAH) demonstration.

    This brief provides comprehensive profiles of four house call programs, some of which participate in IAH. While each operates uniquely, they all seek to accomplish similar goals by leveraging care in the home to meet the needs of patient populations that traditional clinical operations struggle to manage.
