The clinical practice approval form is part of Banner Health’s process for vetting clinical standards before they are implemented system-wide. A representative from the Clinical Consensus Group who helped define the standard will share the completed form at a Care Management Council meeting, with CMOs, CNOs, and other clinical leaders from across the system in attendance. At the meeting, the council will decide to approve the standard for system adoption, or to send the standard back to the author for revisions.
A standard approval form ensures the author has sufficiently vetted the standard against the relevant evidence and other experts before asking for final approval. The form also ensures the suggested standard is presented in a succinct, efficient manner. Download the approval form to support the clinical standard vetting process at your organization.
