Step 2: Decide whether your cancer program should work toward a basic or advanced survivorship program—and on what timeline

Step 3: Engage key survivorship stakeholders—such as cancer program physicians, navigators, social workers, primary care physicians, and survivors themselves—to identify the services that are most needed in the short and long run

Step 4: Select specific program elements from the grid to develop or enhance
