The Patient Care Partner Program

Caregivers often make assumptions about the level of care patients' friends and family can—and want—to provide at home.

To better empower friends and family in patient care, Sharp Memorial Hospital developed a Care Partners Program where they could opt in to and become a formal part of care team. Nurse leaders share the impact their program had on the care provided and the patient experience.
