The growth of ambulatory care continues to explode as health care reform efforts continue to move toward less costly alternatives to hospital-based treatment. Yet ambulatory care presents a different set of challenges around quality and safety issues, heightened by the necessities of managing transitions to and from other settings and coordinating multiple care needs.
In this session, Dr. Ann Scott Blouin, Executive Vice President of Customer Relations,
Joint Commission, describes challenges emerging in ambulatory care, transitions of care and care coordination, and several strategies for health care teams to consider.
Presenters: Dr. Ann Scott Blouin and Katherine Virkstis
