IT in the Post-Acute Care Environment

Health care reform incents acute care providers and post-acute care (PAC) organizations to collaborate to improve the management of patient care across a succession of different locations.

In this research brief, we examine some of the unique information requirements and challenges of the PAC environment and the role and status of the EMR in PAC. We then review the potential role for telemedicine in the PAC space, the management of information flow as patients move between entities, and the importance of establishing some baseline competencies for PAC analytics.
