As organizations increasingly focus on care management, it is important to address unmet health care needs with primary care to prevent avoidable hospital admissions.

But identifying where to start in the outpatient setting can be challenging. Using metrics for ambulatory care-sensitive conditions—conditions for which early primary care intervention can prevent hospitalization or more severe disease—this toolkit helps population health managers target the conditions that are either highly prevalent or poorly managed in their local area or facility.
