The Pay-for-Performance (P4P) Map displays estimated net impact on Medicare revenues as a result of CMS's three mandatory P4P programs for hospitals paid under the inpatient prospective payment system (IPPS). Hospital Readmissions Reduction Program (HRRP), Value Based Purchasing (VBP), and Hospital Acquired Conditions (HAC) data represents all final adjustments as published by CMS to date. The estimated financial impact presented for each facility is derived from the publicly available data from CMS's supplemental files for Readmissions and VBP, which can be found in final rule tables 15B, 16B, respectively, as well as Hospital Compare for HAC flags. We then combine these final adjustment factors with the every organization's latest revenue data from MedPAR to estimate base operating payment amounts for each fiscal year.

Explore trended P4P performance going back to 2013 for every participating hospital in the country. This map is color-coded by severity (i.e. red represents a harsh penalty, while blue represents a significant net bonus) and excludes hospitals located in Maryland, Puerto Rico, and all other U.S. territories that are not reimbursed under IPPS. Select 'Download National Map Data' to get the entire national dataset powering the map. You can also open the map in a new browser to view it in full size.
