About the Webconference

The anticipated influx of heart failure cases presents significant challenges to cardiovascular departments: coordinating care across multiple specialists, carefully transitioning patients out of the inpatient setting, and long-term follow-up. Additionally, the initiation of the Readmissions Reduction Program emphasizes the importance of caring for these patients beyond the acute inpatient stay.

In response, progressive CV programs are building a patient-centered infrastructure to support heart failure care from diagnosis through follow-up. This webconference provides operational guidance on developing heart failure programs and optimizing treatment with implantables and ventricular assist devices (VADs) through profiles of successful organizations.
