Organisations that are new to population health often get bogged down with a long list of priorities. To truly transform care delivery, every population health manager must first develop three key competencies:
- Robust care management. That means bringing patients into the care plan from the start, adopting team-based care models, and taking advantage of new technologies.
- Provider engagement. All members of the care team must be well-versed in standards of care, and build specific care pathways for everything from diabetes management to knee replacements.
- Plan management. To manage the total cost of a population, you’ve got to start thinking like insurers, creating value-based benefits that reward more cost-effective care settings.
