What does it mean to transform care?

Successful population health managers start with robust care management, provider engagement, and plan management.

Organisations that are new to population health often get bogged down with a long list of priorities. To truly transform care delivery, every population health manager must first develop three key competencies:

  • Robust care management. That means bringing patients into the care plan from the start, adopting team-based care models, and taking advantage of new technologies.
  • Provider engagement. All members of the care team must be well-versed in standards of care, and build specific care pathways for everything from diabetes management to knee replacements.
  • Plan management. To manage the total cost of a population, you’ve got to start thinking like insurers, creating value-based benefits that reward more cost-effective care settings.
