The First Step Is Knowing Where You Are

A recent Health Affairs study indicates that the incidence of adverse events (AE) is worse than reported in the IOM’s 1999 report, 'To Err Is Human.' One reason for a lack of progress may be that AE detection methods used by most hospitals do not give an accurate picture of the amount of harm occurring to patients. Better methods are available and should be adopted. IT can help.
