Two thirds of the institutions we work with have formal clinical documentation improvement programs, defined by a dedicated staff that reviews patient charts for accuracy and completion while the patient is still in the hospital. Although there is a wide range of performance levels, the strongest programs focus their efforts on the areas with the greatest opportunity for improvement.
